Legal Guardianship Certificate
Application for Legal Guardianship Certificate under the National Trust Act
The Legal Guardianship Certificate is issued under the provisions of the National Trust Act, 1999 for persons with the following disabilities:
- Autism
- Cerebral Palsy
- Mental Retardation
- Multiple Disabilities
Citizens can apply online for the Legal Guardianship Certificate through the National Trust’s official website.
Required Documents
Please ensure the following documents are ready in scanned PDF format for uploading:
- Disability Certificate (issued by a competent Medical Authority)
- Aadhaar Card of the person with disability (PWD)
- Aadhaar Card of proposed guardian(s)
- Joint Photograph of the person with disability with proposed guardian
- Birth Certificate or any age proof of the person with disability
- Residence Proof (Voter ID, Utility Bill, Aadhaar, etc.)
- Details of Property (Movable / Immovable)
- Affidavit (as prescribed under National Trust guidelines)
Note: All documents should be self-attested by the guardian(s) before uploading.

Citizens can apply through the official Niramaya/National Trust Portal:

- Register/Login as a citizen
- Select “Legal Guardianship” under the available services
- Fill out the required form and upload necessary documents
- Submit the application for processing by the Local Level Committee (LLC)
