Application for Ultrasound Clinic Registration Certificate

(Under the PC & PNDT Act, 1994)

As per the provisions of the Pre-Conception and Pre-Natal Diagnostic Techniques (Prohibition of Sex Selection) Act, 1994, it is mandatory for all Genetic Clinics, including Ultrasound Clinics, to be registered under the PC & PNDT Act before commencing any diagnostic or imaging services related to pre-natal sex determination.

Required Documents for Registration

Applicants (owner/authorized person) must submit the following documents along with the application to the Appropriate Authority (District/State Level):

  1. Application in Form A (as per PC & PNDT Rules, 1996)
  2. Proof of Ownership/Lease Agreement of the premises
  3. Registration Certificate of the Clinic (Trade License/Clinical Establishment License)
  4. Details of Equipment (Invoice, make, model, serial number of Ultrasound machines, etc.)
  5. Affidavit on Non-violation of PC & PNDT Act provisions
  6. Copy of Qualification Certificates of the Sonologist/Radiologist/Qualified Persons
  7. Registration Certificate of Doctors with the Medical Council of India/State Council
  8. Identity and Address Proof of Owner and Medical Practitioners
  9. Declaration of Availability of Records (Form D, E, F, G registers as applicable)
  10. Photographs of the premises (interior and exterior)
  11. Fee Receipt (as prescribed by the Appropriate Authority)

Note: All documents should be self-attested and submitted in hard copy. Any incomplete or false documentation may lead to rejection or cancellation of registration.

Submission Process

  • The application along with all required documents must be submitted to the Appropriate Authority (District Magistrate & Collector PC & PNDT).
  • The clinic shall be physically inspected by the authority before registration.
  • Upon verification, a Registration Certificate in Form B will be issued, valid for 5 years (renewable).
